Community nursing
Nursing care that comes to your home
Diamond Care Victoria is registered with the NDIS Commission to deliver Community Nursing Care — nursing supports provided in your own home across the City of Casey and Cranbourne. Care is planned around your health needs and your routine, and coordinated with your GP and treating team. Call 1300 765 559 to discuss what you need.

Why at home
Health care shouldn’t require a waiting room
For many people with disability, the hardest part of routine health care is the logistics around it — the transport, the transfers, the waiting. Community nursing moves the care to you: delivered at home, scheduled around your life, by a service registered and audited for exactly this support.
How it works
How community nursing works with us
It starts with your health needs, not our roster. We gather what your treating team has set out — GP instructions, specialist guidance, hospital discharge plans — and build nursing support that follows it precisely.
Your nursing care coordinates with the rest of your supports, so the left hand knows the right hand’s schedule: personal care, medication routines and nursing visits run as one plan, not three.
What to expect
What to expect
Nursing supports delivered in your home, at agreed times. Clear records of what was done and what was observed, shared appropriately with your treating team. Escalation that works: if something changes in your health, the pathway to your GP or to emergency care is named in your plan, not invented on the day. And communication in the form you prefer — to you, your family or your nominee, as you direct.
FAQs
Questions people ask us
Are you registered to provide nursing care?
Yes. Community Nursing Care is on our NDIS registration, approved to 9 October 2028 — verifiable on the NDIS Commission’s public register.
How does nursing get funded in my plan?
Community nursing is typically funded where a disability-related health need requires it. We’ll help you and your coordinator confirm what your plan covers before anything starts.
Do you work with my GP and specialists?
Yes — nursing support follows your treating team’s guidance, and records are shared with them as you consent.
Can nursing visits line up with my other supports?
That’s the point of one provider: nursing, personal care and household support scheduled as one coherent plan.
