Transitions are where good arrangements break

A support setup that works beautifully can fail in a fortnight when circumstances turn: school ends and the weekday structure vanishes; a hospital stay rewrites what home needs to provide; a parent who carried the invisible load can’t anymore. Transition support exists to get ahead of these moments — mapping what changes, what must be in place before it does, and who does what by when.

How we work a transition

Map the change and its ripple effects — the obvious ones and the ones two steps out. Build the sequence: what’s needed, in what order, by when, owned by whom. Coordinate the moving parts — providers, funding, family, services. Walk it with you at the pace the change demands. Then review on the other side, because the plan that got you through the corner isn’t always the plan for the straight.

Questions people ask us

When should transition planning start?

Earlier than feels necessary. School leavers: start of the final year, not the final term. Discharges: as soon as a date is discussed. Six months of planning beats six weeks of scrambling, every time.

What funding does this use?

Capacity Building — support coordination or related categories depending on your plan. We’ll map it before starting.

Can you coordinate with the school / hospital / other providers?

That’s most of the job. With your consent we work directly with every party in the transition, so you’re not the messenger between them.

What if the transition is already happening — or already went wrong?

Call. Stabilising a transition mid-flight is harder but absolutely doable, and faster with someone who’s run the pattern before.
