Support coordination is NDIS-funded help to understand your plan, choose providers and get supports actually running. Diamond Care Victoria delivers support coordination across the City of Casey and Cranbourne. Because we also deliver direct supports, we tell every participant plainly: you are never required to use us for anything else. Call 1300 765 559.

The role
What a support coordinator actually does
Reads your plan with you and explains what each budget can and can’t buy. Finds and compares providers — and books the meetings. Sets up service agreements and bookings. Coordinates between your providers, your GP, allied health and family so you aren’t the messenger. Steps in when something breaks: a provider folds, a discharge lands, housing shifts. Prepares evidence for your plan reassessment ahead of time, not the week before. And builds your capacity to run more of this yourself — which is the actual point.
Transparency
The conflict-of-interest conversation, up front
We deliver both support coordination and direct supports. That’s a real conflict of interest, and pretending otherwise would insult your intelligence.
So, in writing:
- Your coordinator presents at least two options wherever alternatives exist.
- Choosing us for one support never obliges you to choose us for another.
- If you’d prefer a fully independent coordinator, say so and we’ll refer you to one — we’d rather do that than hold a plan we shouldn’t.
- Your coordinator’s notes and recommendations are yours to see on request.

Coordinating a discharge? Call, don’t form. We work with discharge planners and social workers on short timelines, and we’ll tell you within one business day whether we can take it — including when the answer is no, so you’re not chasing.

Questions people ask us
Can I use you for coordination and someone else for everything else?
Yes — many participants do exactly that, and our conflict-of-interest commitments are built for it.
Is support coordination in every plan?
No — it’s a Capacity Building item that has to be funded. If it’s not in your plan and you need it, we can help you build the case for your next reassessment.
Can I switch coordinators?
Any time. If you’re leaving us, you’ll get a clean, complete handover — the same one we’d want to receive.
Do you charge travel?
Per the NDIS Pricing Arrangements, explained in your service agreement before you sign.
Which suburbs?
City of Casey and Cranbourne core; Cardinia and Greater Dandenong subject to capacity.
