High intensity support

Complex personal care, matched to proven competency

Diamond Care Victoria is registered with the NDIS Commission to deliver High Intensity Daily Personal Activities — personal care where complex health needs require additional worker training and clinical oversight. We serve the City of Casey and Cranbourne. Tell us what you need and we'll confirm at assessment whether we can deliver it safely.

Definitions

What makes support “high intensity”

Some daily supports carry clinical complexity — the kind where the worker’s competence isn’t optional and “close enough” isn’t a standard. The NDIS defines these as high intensity supports, and delivering them requires a higher registration standard, which we hold: workers trained against the NDIS high intensity support skills descriptors, with clinical oversight behind them.

Our process

How we decide what we can deliver — honestly

We don’t publish a menu of complex procedures, and that’s deliberate. High intensity support is safe when a specific worker’s assessed competency matches a specific participant’s needs — not when a website promised something in general. So our process is:

  1. 1

You tell us what you need

Or your coordinator, nurse or discharge planner does.

  2. 2

    We assess

Your support needs, the clinical requirements, and the competencies of the team we’d roster.

  3. 3

    We answer plainly

Yes with a support plan, or no with a referral. What you’ll never get from us is a yes that outruns our training.

Safeguards

What sits around every high intensity support

  • A written support plan reflecting health practitioner guidance
  • Workers trained and assessed against the relevant skills descriptors before their first shift with you — and refreshed after it
  • Clinical oversight and escalation pathways that are documented, named and tested
  • Incident reporting that actually gets reviewed
  • The same choice and dignity standards as every other support we run: your consent, your preferences, your right to change workers

FAQs

Questions people ask us

Are you registered for high intensity supports?

Yes — High Intensity Daily Personal Activities is on our NDIS registration, approved to 9 October 2028 and verifiable on the Commission’s public register.

Which complex supports can you deliver?

It depends on matched competency, so we confirm at assessment rather than promise on a webpage. Tell us your needs and you’ll get a straight yes or no within days, not a maybe that drifts.

Who oversees the clinical side?

Supports are delivered under documented clinical oversight, with health practitioner guidance built into each support plan and named escalation pathways.

Can you take a hospital referral?

Yes — discharge planners should call 1300 765 559. Assessment starts from your paperwork, and we confirm capacity within one business day.

What if my needs increase over time?

Your support plan is reviewed, worker competencies are reassessed, and the roster changes with you. If your needs move past what we can deliver safely, we say so and help you transition — before there’s a gap, not after.

Tell us what you need. We respond within one business day.

Call the office, or send the request form and we’ll confirm your funding pathway, your suburb and whether we can genuinely deliver what you need. If we can’t, we’ll say so and point you to someone who can.
